Below are the forms you will need to return to Little Geeks to register your child for our program.

Application Forms:

Choose one of the two forms below and fill out an application for your child. Click to open or right click if you have Windows to save it to your desktop.

  • Application for a Computer - Microsoft Word format

    • This form can be opened and printed with the Microsoft Word program that comes in Microsoft Office. If you do not have Microsoft Word, you can download for free a Microsoft Word viewer from the Microsoft web site.

  • Application for a Computer - PDF format

    • This form can be opened and printed with the Adobe Acrobat or Adobe Reader program. If you do not have either, you can download Adobe Reader for free at Adobe Reader from the Adobe.com web site.

Household income will be taken into consideration on a case by case basis when reviewing applications for computers.

Child must be between the ages of 8-17 years old.

For now the child must live in the Greater Toronto Area.

Please fill all forms out as completely as possible. When asked please also state the reasons why the child would benefit from a computer and what other community agencies the child is connected with.
